This form is for parents who are interested in having their child age 0-6 years baptized at Our Lady of Angels.

You will be contacted to schedule a visit at the church to get to know you and go over expectations.

If the child has not been born yet:
     * Enter the child's last name, but put "TBD" for the first name.
     * Leave Date of Birth blank.  In the Comment box at the bottom, put the approximate due date.
