NOTICE REGARDING RIDESHARE AND THIRD-PARTY TRANSPORTATION SERVICES

Our Lady of Angels Catholic Parish strongly discourages the use of rideshare services or any third-party transportation providers for the transportation of minors to and from parish-sponsored events and activities. Such services are not affiliated with, endorsed by, or supervised by the parish, and their use may present safety risks beyond the parish’s control.

In the rare circumstance that a family elects to utilize such a service, an “Ongoing Parental Consent and Release of Liability for Rideshare” form must be fully completed, signed, and on file with the parish no later than August 25, 2025, at the beginning of the Faith Formation year.

Important:

- This form MUST be requested from either the Director of Youth Ministry and/or the Youth Ministry Coordinator.

- This consent form is valid only for the period of July 2025 through July 2026.

- A new form must be submitted for each subsequent program year.

Without a current, completed form on file, no minor will be permitted to arrive or depart from parish events via rideshare or third-party transportation services.


Our Lady of Angels Catholic Parish remains committed to ensuring the safety and well-being of all minors entrusted to our care. We sincerely urge families to make transportation arrangements that prioritize the direct supervision and safety of their children.
This form is ONLY for paying the registration fee for the Dallas Catholic Youth Conference.  It is NOT for registering.

If you register before January 1, 2026 the price is $210. After January 1st, 2026 the price will be $230. 

Payment must be made by January 16th, 2026. After Janurary 16th, 2026 all payements will increace to $230 until Janurary 1st, 2026. 

No one will be turned away due to payment. Please reach out if you need financial assistance. 

Cancellations must be received by January 16, 2026 in order to receive a full refund.  Canceliations by Feburary 8, 2026, will receive a partial refund. Cancellations after Feburary 8, 2026 are not refundable unless you can arrange for a substitution of the same gender by Feburary 8, 2026.
